Newer approaches to endoscopic excision of angiofibroma- The push-pull and the four-port approach

Abstract:
Angiofibroma (JNA) is a benign but locally invasive, highly
vascular tumour which affects young boys around
puberty. Endoscopic excision of this tumour is now a widely
accepted mode of treatment.
This lecture details the recent advances in surgical techniques
which can be used to excise large tumours with far lateral
extensions. Two new techniques are described: 1) The Push-
Pull approach and 2) The Four-Port approach
The Push-Pull technique uses two ports – an Endonasal port
and a Trans-oral port to dissect and deliver large tumours from
the infra-temporal fossa. The Four- Port Technique is a further
refinement of the Push-Pull technique. In this technique the
surgeon uses four separate ports to excise the tumour viz. the
contralateral nostril, the ipsilateral nostril, the canine fossa
and a trans-oral port. The main advantage of these techniques
is that large tumours can be excised endoscopically without
damaging the pyriform aperture and the anterior facial
skeleton as is done in the Modified Endoscopic Denker’s or
Midfacial Degloving approaches.
The lecture includes discussion on advantages, disadvantages
and limitations of these two approaches. A long term follows
up of up to 4 years of these patients is also presented.
Biography:
Renuka Bradoo is Professor & Head of Department of Otorhinolaryngology at LTM Medical College and General Hospital, Mumbai, India. She is an internationally acknowledged expert in the field of endoscopic sinus and skull base surgery with her special interest and focus of research being Angiofibroma. She was first surgeon in India and amongst the first in the world to undertake endoscopic excision of angiofibroma. She has over 70 national and international publications to her credit. Her published work on ‘Transcutaneous Orbital Endoscopy’ for lesions lateral to the optic nerve is amongst the first few such reports in world literature. She is the author of ‘Anatomical Principles of Endoscopic Sinus Surgery – A Step by Step Approach’. She has conducted an annual course in Endoscopic Sinus Surgery at her institution for the last 14 years during which she has trained over 1100 ENT surgeons from India and abroad.
