A rare case of sinonasal mucosal melanoma

Abstract:

Sinonasal mucosal melanoma is a rare tumour representing less than 1% of all melanomas and less than 4% of all Sinonasal neoplasms. Nasal obstruction and epistaxis are the most commonly reported symptoms. These symptoms may develop late and usually nonspecific, therefore it tends to delay the diagnosis and result in a poorer prognosis. We report a case of Sinonasal mucosal melanoma in an 84-year-old female who presented with nasal obstruction and epistaxis. Computed tomography of the facial sinuses revealed a large lesion in the right nasal cavity, extending to the left nasal cavity, the left nasopharynxand destroying the medial wall of the bilateral maxillary sinuses and nasal septum. The patient underwent extirpation of the mass with the left lateral rhinology approach, right endoscopic sinus surgery and right anterior orbitotomy by the ophthalmologist. The post-operative diagnosis of Sinonasal mucosal melanoma was confirmed by histopathology examination along with immunohistochemistry by using S100 and HMB 45. Sinonasal mucosal melanoma is a rare tumour, with aggressive behaviour and poor prognosis. Rarity of this lesion warrants its mention and emphasizes the importance of considering Sinonasal mucosal melanoma among the differential diagnosis of tumours of nose and paranasal sinuses.

Valentina Dian Juwitawati, MD is a graduate of faculty of medicine, public health and nursing universitas gadjah mada, yogyakarta, Indonesia. She had worked as general practitioner at emergency department of panti rapih hospital, yogyakarta, Indonesia for eight years. Currently, she undergoes residency at Otorhinolaryngology Head and Neck Surgery department, faculty of medicine, public health and nursing universitas gadjah mada/Dr. Sardjito General Hospital, yogyakarta, Indonesia.
